What are MTProto proxies?
MTProto is Telegram's own transport protocol, and an MTProto proxy is a server that speaks it. Your client wraps the session so that anything watching the wire sees a TLS handshake to an ordinary web host rather than a recognisable Telegram connection — which is why this type gets through deep-packet inspection that blocks the app outright.
Telegram supports it natively: there is no client to install and no VPN in the path.
You can find free ones in about ten seconds, and thousands of people do, so the honest question is what the paid one adds. Three things.

How MTProto proxies work
The proxy sits between your client and Telegram's data centres and re-encrypts the session with a secret you are given. The disguise is the point: our endpoints answer on 443 behind a TLS front, so to a filtering box the traffic resembles an HTTPS request to a normal website.
Message contents are never exposed by this — they stay encrypted between your client and Telegram either way.
Connecting takes one tap. The order page hands you a tg://proxy link with the host, port and secret already in it; opening it on a device with Telegram installed adds the proxy and switches to it. If you prefer, the same three values can be typed into Settings → Data and Storage → Proxy by hand.
One thing this does not do is move you. Telegram assigns your account to a data centre when you register, and a proxy does not change that, so an MTProto proxy is a way in rather than a way to appear elsewhere.

The operator can see your IP address and that you are connecting to Telegram.
